Registration Form - PDF - This form is required to be completed by NYRA.
CFA Form - PDF - This form is required to be completed by the league. Do not return it as part of your registration material. Once signed by a doctor, return it ASAP.

COACHING AND CLEARANCE FORMS:
Completed forms can be delivered to the appropriate Vice President. They can also be emailed to nyranews@gmail.com
NYRA Coach's Application - PDF - General application to be completed by all coaches.
Act 34 (NYRA Security Clearance Form) - PDF - This is required to complete your criminal background check. All coaches & board members should fill this out.
Act 151 (Form CY-113) - PDF - This is required for your Child Abuse Clearance. All coaches and board members should fill this out.
